Passports
Please bring any existing passport/s to the appointment even though it's full or expired! Please note that there is no queue on the appointment date and therefore we only require 20 - 30 minutes of the applicants time!

Renewal /application of ordinary adult passport

• Copy of identity document (i-d)
• Two passport size photos
• Completed and signed application form (for child application, both parents must sign section c of application)
• Temporary passports can only be issued if and when an ordinary passport is applied for at the same time. (temporary and ordinary requires four photo’s). back to top

Temporary passport additional form to be completed

• In order to obtain the temporary passport a company/ instutution/valid personal reasons letter must accompany the application explaining the reason and date of urgent travel
  *example: i mr…. / mrs.… need to apply for a temporary passport. The reason for this is that my ordinary passport won’t be ready when i depart for (country) on (date of travel). Your assistance in this urgent matter is much appreciated
• The processing time for a temporary passport can take between 1-5 days. Depending on travel date. back to top

Maxi (double- 48 pages) passport

• One application form and two passport size photos are required
• Original bar-coded identity document
• Existing passport
• The processing time for an ordinary/maxi passport can take between 10-12 weeks. back to top

Second passport

• One application form and two photos
• A letter on a company letterhead explaining the reason for obtaining the second passport.
  *example:"at times the one passport is in an embassy/ consulate and then mr. / mrs.____ urgently requires travelling to another country”

Child passports (age 1 year to 16 years)

• One application form and two passport size photos are required. (please make sure both parents sign section c on application  : that is parental consent)

Birth

• Register a birth (within 30 days after the birth) application for a birth certificate
• Register a birth (late registrations): after 30 days, but before one year

Unabridged marriage certificate

• Copy of husband and wife's identity document and copy of the marriage registrar (obtainable from the church or home office where the applicant was married). back to top

• Application for a death certificate, Abridged or unabridged 6-8 weeks (full)

Identification

initial issue of an identity document 2 months

Issued to:
citizens at the age of 16 years and older and to persons to whom permanent residence has been granted.
A certified copy of the birth certificate and/or a reference book number, or, if applicable, a copy of the tbvc-countries' identity or travel document should be submitted.

If a person is married, the marriage certificate must be submitted. Two photographs

re-issue of an identity document

Documentary proof of i-d number or affidavit if stolen/lost, two photographs.
Should a woman apply for the reissue of an identity document and wants to resume any of the surnames she was previously known by, documentary proof must be submitted to prove that she is entitled to that surname.

Temporary identity document

Two photographs (can only be issued when a ordinary i-d has been applied for at the same time). back to top

Amendments

change forename(s)

change surname of a minor

Due to error:

• Copy of both parents identity documents or submission of proof of the correct information is a prerequisite in such instances, applicants birth certificate [  special cases  must submit the following which ever is applicable]
• Both the natural parents written consent is required, as well as a good and sufficient reason, in writing, for the change.
• The natural father's written consent, unless waived by a competent court is a statutory requirement in the case where the child was born in wedlock. 
• The husband whose surname the child is to assume must also give his written consent to the assumption.
  Copy of birth certificate
• Both natural parents i-d copies
• Legal guardian id copies (in case of adoption)
• Courts letter stating relationship (where applicable). back to top

Due to error:

• Submission of proof of the correct information is a prerequisite in such instances – [that is parents or next of kin identity document, etc.]
• Original incorrect identity document and copy of [for a person who has not entered into a legal marriage]

Special cases submit the following:

• Copy of identity document
• Applications must be on a duly completed bi-196 form and a good and sufficient reason, in writing, for the change must be furnished
  -assuming maiden surname or a prior surname she legally bore
• Divorce decree
  Identity document copy
  - assuming double-barrelled surname [joining her surname with that of her husband's]
• Marriage certificate

Change gender

For persons undergone a gender reassignment procedure

• Two medical reports
  • one by the medical practitioner who applied the procedure
  • a report by a second medical practitioner who has independently examined the applicant

For intersexes persons

• report by a medical practitioner corroborating that the applicant is intersexed.
• report by qualified psychologist or social worker corroborating that the applicant is living and has lived stably and satisfactorily, for an unbroken period of at least two years in the gender role corresponding to the sex description under which he or she seeks to be registered. back to top
